Pre-Surgical Psychological Evaluations

These evaluations are for individuals considering invasive pain procedures (e.g., spinal cord stimulators, intrathecal pump). They ensure that you’re mentally and emotionally prepared for the journey ahead by assessing your expectations, readiness, and recovery strategies.

Usually, you’ll receive guidance from your medical provider when it’s time to pursue the psychological evaluation. We would be happy to work with you and your referring provider to complete your evaluation virtually. You can let your provider know your preference to work with us, or you can complete the self-referral form below and we’ll contact them on your behalf.

We’ll use this authorization to reach out to them and confirm your medical approval for the procedure and any other next steps, including whether a pre-surgical psychological evaluation is right for you at this time.

Clicking the above button will take you to a separate, secure website.

Please note, you must complete email verification to get access to the form. All information shared on the form is transmitted via our HIPAA-compliant electronic health record, PracticeQ.
